How to I submit a quarterly affidavit in SLIP+ for States?

Quarterly affidavits must be submitted using the agent’s primary SLIP+ for States account. Log into the account and select Quarterly Affidavits under the COMPLIANCE tab. Select the State and Quarter for the affidavit you need to submit. Type in the Agent’s First and Last Name, click submit.
